Metformin against Berberine

Berberine is sold as a natural alternative to metformin, and unusually for a supplement claim there is a direct trial behind the comparison. It is small, it is old and it was run in one country, all of which matters as much as the result.

In study A the authors report that the hypoglycaemic effect of berberine was similar to that of metformin. In the berberine group HbA1c fell from 9.5 to 7.5 percent, fasting glucose from 10.6 to 6.9 mmol/L and postprandial glucose from 19.8 to 11.1 mmol/L, all p below 0.01. Across the paper, 20 of 58 patients, 34.5 percent, had transient gastrointestinal adverse effects, and no liver or kidney damage was seen.

What the trial was

A pilot randomised trial. 36 adults with newly diagnosed type 2 diabetes were assigned to berberine or to metformin 0.5 g three times a day for three months. The paper also reports a separate single-arm study B in 48 adults with poorly controlled diabetes, which had no metformin comparator.

What the authors concluded

The authors conclude that berberine is a potent oral hypoglycaemic agent with beneficial effects on lipid metabolism, and describe study A as a pilot.

Where this result is weak

Eighteen people per arm for three months is a pilot, not a basis for substitution, and the paper says so. The metformin dose was 1.5 g a day, below the 2 g commonly used. There was no placebo arm, no blinding described, and the endpoints were glucose markers rather than any complication, hospitalisation or death. Metformin's evidence base includes outcome trials running for years; nothing comparable exists for berberine.
