| Mechanism | Coenzyme Q10 is an endogenous lipid-soluble electron carrier in the mitochondrial inner membrane, shuttling electrons from complexes I and II to complex III in the respiratory chain. In its reduced form, ubiquinol, it is also a lipid-phase antioxidant that regenerates vitamin E. Statins inhibit HMG-CoA reductase, which sits upstream of both cholesterol and CoQ10 synthesis in the mevalonate pathway, and that shared pathway is the mechanistic rationale for the statin myalgia hypothesis. The rationale is sound and the trials still came back null, which is a useful reminder that a coherent mechanism is not a result. | A redox cofactor that cycles between oxidised and reduced states and can act as an antioxidant and as a signalling molecule. The proposed route is activation of AMPK and upregulation of PGC-1alpha, the transcriptional coactivator that drives mitochondrial biogenesis, which has been shown in a rotenone rodent model of Parkinson's disease and in mouse work.
