Head to head

Zoledronic acid against Teriparatide

The two ends of osteoporosis treatment, a once-yearly infusion that slows bone loss and a daily injection that builds bone. Zoledronic acid also carries the most interesting unexplained mortality signal in geroscience, 9.6 percent against 13.3 percent on placebo after hip fracture, which a meta-analysis of 38 osteoporosis drug trials did not confirm.

Column A

Zoledronic acid

Zoledronic acid, an intravenous nitrogen-containing bisphosphonate

An approved once-yearly bone drug carrying the most interesting unexplained mortality signal in geroscience: 9.6 percent died on zoledronic acid against 13.3 percent on placebo in a trial after hip fracture, a secondary finding never satisfactorily explained. A meta-analysis of 38 osteoporosis drug trials then found no mortality benefit.

Column B

Teriparatide

Teriparatide (parathyroid hormone 1-34)

An FDA-approved bone-building peptide, a fragment of parathyroid hormone used for severe osteoporosis.

AttributeZoledronic acidTeriparatide
CategoryLongevityHormone
FDA statusFDA-approved. Marketed as Reclast for osteoporosis in postmenopausal women and men, glucocorticoid-induced osteoporosis and Paget disease of bone, and as Zometa for hypercalcaemia of malignancy and bone metastases. It is not approved for ageing, longevity or mortality reduction, and no such indication has been sought.FDA-approved (Forteo) for osteoporosis
Half-lifePlasma clearance is rapid and multiphasic, but the drug binds to bone mineral and is released slowly over years as bone turns over, so no single plasma figure describes its duration of action. This is why it is dosed once a year.~1 hour
Molecular weight272.1 Da for the anhydrous acid.4,117.8 Da
MechanismZoledronic acid binds avidly to bone mineral and is taken up by osteoclasts, where it inhibits farnesyl pyrophosphate synthase in the mevalonate pathway. That blocks prenylation of small GTPases the osteoclast needs, and the osteoclast stops resorbing bone and dies. This is the established mechanism and it explains the fracture result completely. It does not explain the mortality signal. Proposed non-skeletal mechanisms include a senolytic or senomorphic action, supported by Mayo Clinic work showing selective killing of senescent cells in vitro and reduced circulating SASP factors and improved grip strength in aged mice; effects on pre-osteoclastic cells that carry senescence markers; and general effects of the mevalonate pathway on immune cells. None has been shown to account for the observed survival difference in people.Intermittent activation of PTH receptors stimulates bone-forming osteoblasts, increasing bone density and strength.

Frequently asked questions

What is the difference between Zoledronic acid and Teriparatide?

Zoledronic acid: An approved once-yearly bone drug carrying the most interesting unexplained mortality signal in geroscience: 9.6 percent died on zoledronic acid against 13.3 percent on placebo in a trial after hip fracture, a secondary finding never satisfactorily explained. A meta-analysis of 38 osteoporosis drug trials then found no mortality benefit. Teriparatide: An FDA-approved bone-building peptide, a fragment of parathyroid hormone used for severe osteoporosis.

Which has stronger research evidence, Zoledronic acid or Teriparatide?

This database does not grade compounds. It counts what was run in people: 4 of the 6 studies cited on the Zoledronic acid profile were human work, against 2 of 2 for Teriparatide. Those counts are of our own citation lists and understate any larger literature, and neither is a recommendation.

Are Zoledronic acid and Teriparatide FDA-approved?

Zoledronic acid: FDA-approved. Marketed as Reclast for osteoporosis in postmenopausal women and men, glucocorticoid-induced osteoporosis and Paget disease of bone, and as Zometa for hypercalcaemia of malignancy and bone metastases. It is not approved for ageing, longevity or mortality reduction, and no such indication has been sought.. Teriparatide: FDA-approved (Forteo) for osteoporosis.
