A C-type natriuretic peptide analogue approved for achondroplasia on a randomised phase 3. It increases growth velocity by roughly 1.6 cm a year, and nobody knows the effect on final adult height.

Vosoritide is the clearest example on this site of a peptide designed from a precise understanding of a single mutation. Achondroplasia is caused by a gain-of-function mutation in FGFR3 that over-suppresses growth plate cartilage.

Mechanism: A 39-amino-acid analogue of C-type natriuretic peptide, modified to resist degradation by neutral endopeptidase, which destroys the native peptide within minutes. It binds natriuretic peptide receptor B, raising cyclic GMP and activating protein kinase G, which inhibits the RAF and MEK and ERK cascade at the level of RAF-1.

Regulatory history: FDA-approved as Voxzogo to increase linear growth in children with achondroplasia with open epiphyses. Approval rests on growth velocity, not final adult height.

Registers or lists therapeutic goods on the ARTG and schedules substances in the Poisons Standard. Many peptides are Schedule 4 (prescription only), so possession without a prescription and personal importation are restricted even when no product is registered.
