When homocysteine is high
Homocysteine is the marker the owner's methylated-B rule waits on. B vitamins can lower it. HOPE-2 and NORVIT then showed that lowering it did not deliver the vascular-event benefit people wanted.

The coach gate
Check B12 with MMA, and check folate, before you add folate. Treating folate first can hide B12 deficiency. MTHFR is context, not a protocol.
What this does not mean
It does not mean a methylated B-complex prevents heart attacks or strokes. The large trials moved the number and not the events. It does not mean everyone with an MTHFR variant needs a bottle.

Candidate supplements, not orders
What people typically order
Ask for homocysteine, serum B12, MMA, and folate. A clinician decides whether MTHFR testing adds anything. Do not start high-dose folate alone.
Stopping rule
If you and a clinician trial B vitamins, retest homocysteine at about ninety days. If it did not move, stop. If it moved, remember that the outcome trials still failed. Do not take movement as a saved life.
