When 25-OH D is low
A low 25-hydroxy vitamin D means you are in the group the huge prevention trials mostly did not enroll. Repletion is a real clinical conversation. It is not proof that a bottle will prevent cancer or death.

The coach gate
Confirm the unit (ng/mL or nmol/L). Ask whether calcium and PTH belong beside a very low value. Pregnancy, malabsorption, and kidney disease change the plan and are clinician gates.
What this does not mean
It does not mean VITAL was wrong, and it does not mean 2,000 IU will prevent a heart attack. It does not mean an annual mega-dose is smarter. That design increased falls and fractures.

What people typically order
Ask for 25-hydroxy vitamin D. If the clinician is considering a higher dose, ask for calcium. K2 is often paired on the owner's floor list. That pairing is a stack choice, not a trial mandate.
Stopping rule
Retest at about ninety days. If the number did not move, stop guessing the dose in private. If you are already in the 40 to 60 ng/mL conversation band the owner uses, more is not automatically better.
