Klotho Explained: Protein, Peptide, Gene Therapy or Supplement?

What is klotho and can you buy it?

Klotho is an anti-aging protein made mainly in the kidney and brain; its secreted form, s-KL, falls with age. No approved klotho drug exists. A 2025 mouse study extended lifespan 19.7 percent with a single gene therapy dose, and a 21-person Phase 1b mRNA trial began in 2026. Products sold as 'klotho peptide' are research reagents or unrelated peptides.

The name comes from the Greek Fate who spins the thread of life, and it was earned: in 1997 a team led by Makoto Kuro-o found that mice with a disrupted klotho gene developed arteriosclerosis, osteoporosis, skin atrophy, infertility and short lifespans that looked like accelerated human aging (Nature, 1997). In 2005 the same group showed the opposite: mice overexpressing klotho lived roughly 20 to 30 percent longer (Science, 2005). Few genes have that kind of two-directional evidence, which is why klotho has become one of the most discussed molecules in longevity circles.

What klotho does in the body

Membrane klotho is a co-receptor for FGF23, the hormone that controls phosphate and vitamin D handling in the kidney. The extracellular part is shed into blood and cerebrospinal fluid as soluble klotho (s-KL), which acts as a circulating hormone: it dampens insulin and IGF-1 signaling, restrains Wnt signaling, boosts antioxidant defenses through FoxO and Nrf2 pathways, and supports synaptic function in the brain. Its cousin beta-klotho is the co-receptor for FGF21, which is why klotho keeps coming up in discussions of drugs like efruxifermin.

In people, klotho levels are highest in early adulthood and decline steadily. Lower levels associate with chronic kidney disease, cardiovascular events, frailty and worse cognition. Carriers of the KL-VS gene variant, who have modestly higher klotho, have shown better cognitive performance and different brain aging trajectories in several cohorts (Neurobiology of Aging, 2021). These are associations; none proves that raising klotho in an adult changes outcomes.

The 2023 monkey paper is the one that moved klotho from mouse curiosity to human candidate. A single subcutaneous injection of klotho at 10 micrograms per kilogram improved performance on a spatial memory task in aged rhesus macaques, with the effect lasting about two weeks (Nature Aging, 2023). Higher doses did not work better, which is a common pattern for hormones. The 2025 gene therapy study (Molecular Therapy00120-0)) is the first to show a lifespan effect from a treatment given to already-adult animals.

When will klotho gene therapy be available?

No klotho therapy is approved or in late-stage trials. As of September 2026: Klothea Bio started a 21-adult Phase 1b safety trial of AKL003, an intravenous mRNA alpha-klotho, in Honduras in February 2026; Klotho Neurosciences plans an IND for KLTO-202, an AAV gene therapy for ALS, around late 2026 to early 2027; and Minicircle sells an unregulated klotho gene therapy offshore. Any approval is years away.

Offshore gene therapy has no dose control, no long-term follow-up and no regulator. The mouse lifespan data are real; extrapolating a permanent genetic change to yourself from them is not a small step.

What is the 'klotho peptide' sold online?

Native alpha-klotho is a 998 amino acid protein of about 130 kilodaltons. The vials sold as 'klotho peptide' or 'alphaKlothoLR', often around $500 for 20 micrograms, are short engineered fragments or research reagents such as 'klotho-derived peptide 1', a kidney research tool. None has a published human study, and a 20 microgram vial could not deliver the 10 microgram per kilogram dose used in the monkey study to an adult.

Do klotho supplements work? What raises klotho in humans?

No supplement contains klotho. In human studies, higher circulating klotho has been associated with regular aerobic exercise, adequate vitamin D status, good kidney function and, in some cohorts, use of common blood pressure and diabetes drugs. These are associations from observational or small interventional studies; none has been shown to improve outcomes by raising klotho.

The senolytic combination dasatinib plus quercetin raised klotho in mice and in a small human kidney study, which is where the Reddit claim that 'senolytics restore klotho' comes from. It is a mechanistic signal, not a treatment.

The bottom line

Klotho is one of the best-supported aging targets in animals and has zero completed human efficacy trials. That combination attracts both serious biotech money and gray-market products with the same name. If you see the word klotho on a vial, the questions to ask are: is this the full protein, what dose, and where is the human data. In 2026 the honest answers are no, too small, and nowhere yet.
