CagriSema against Semaglutide

The combination against one of its own halves. Both arms ran inside the same phase 3 trial, which is rarer than it sounds, so the question of what adding an amylin analogue buys on top of semaglutide has an answer from one randomised population rather than from two trials stitched together.

Mean body weight fell 20.4 percent on cagrilintide plus semaglutide against 3.0 percent on placebo, a difference of 17.3 percentage points (95% CI 18.1 to 16.6, p < 0.001). Gastrointestinal adverse events affected 79.6 percent of the combination group against 39.9 percent on placebo and were mostly transient and mild to moderate. The published abstract reports the placebo comparison; it does not give a figure for the semaglutide-alone arm.

What the trial was

Phase 3a, 68 weeks, double-blind, placebo-controlled and active-controlled. 3,417 adults without diabetes with a BMI of 30 or more, or 27 or more with at least one obesity-related complication, randomised 21:3:3:7 to cagrilintide 2.4 mg plus semaglutide 2.4 mg (2,108), semaglutide 2.4 mg alone (302), cagrilintide 2.4 mg alone (302) or placebo (705), all with lifestyle intervention. The coprimary endpoints were relative change in body weight and the proportion losing 5 percent or more at week 68, both against placebo.

What the authors concluded

The authors state that cagrilintide and semaglutide given together provided significant and clinically relevant body weight reductions compared with placebo.

Where this result is weak

The semaglutide-alone arm existed but this trial was designed and powered against placebo, not against semaglutide, and it allocated seven times as many people to the combination as to either single agent. A 302 person comparator arm inside a 3,417 person trial supports description rather than a formal superiority claim. It ran 68 weeks, measured weight rather than any hard clinical outcome, and was funded by the manufacturer of both components.
