Semaglutide against Bimagrumab

The muscle question, asked directly. Weight loss on a GLP-1 includes lean mass, and bimagrumab was designed to move fat and muscle in opposite directions. Unusually, the two were randomised against each other and against their combination in the same trial.

Least squares mean weight change at week 48 was 9.3 kg lower on bimagrumab 30 mg per kg, 14.2 kg lower on semaglutide 2.4 mg and 17.8 kg lower on the high dose combination, against 3.3 kg on placebo, all p < 0.001 against placebo. Bimagrumab's common adverse events were muscle spasms, diarrhoea and acne; semaglutide's were nausea, diarrhoea, constipation and fatigue.

What the trial was

Double-blind, placebo-controlled phase 2 trial. 507 adults with obesity, defined as BMI 30 or more, or 27 or more with at least one obesity-associated complication, excluding diabetes, randomised equally to nine groups for 48 weeks: placebo, bimagrumab 10 or 30 mg per kg intravenously every 12 weeks, semaglutide 1.0 or 2.4 mg weekly, and the combinations. An open-label extension ran to week 72. Primary endpoint was absolute change in body weight at week 48.

What the authors concluded

The authors conclude that bimagrumab plus semaglutide produced substantial reductions in body weight, with safety consistent with the known profiles of both drugs.

Where this result is weak

The published comparisons are each arm against placebo, not bimagrumab against semaglutide, so reading the two numbers as a ranking goes beyond what the trial tested. It is a phase 2 of 507 people over 48 weeks with total body weight as the endpoint, which is exactly the endpoint that does not capture bimagrumab's stated purpose, since a drug that adds muscle while removing fat is penalised on the scale.
