This is how I would explain it to a friend. The evidence, with the numbers, is further down the page.
Mazdutide is a once a week shot that turns down hunger and tells the liver to burn fat. It works on the Ozempic hunger signal plus glucagon, the same pair as survodutide. It is approved in China for weight loss and for type 2 diabetes. It is not approved in the US, the UK or anywhere else. Anything sold online outside China as mazdutide is not the drug from the trials.

What studies in people showed
In the big weight trial in China, 610 adults took it for 32 weeks. On the 6 mg dose they lost 12.6 percent of their body weight. On 4 mg they lost 10.1 percent. The dummy shot group gained 0.5 percent. In diabetes, 731 adults got this drug or an older weekly drug called dulaglutide for 28 weeks. Both doses beat it on blood sugar and dropped 3.8 to 5.8 percentage points more weight. Those results got it approved in China in June 2025 for weight and September 2025 for diabetes. The big trials were all in Chinese adults. There is no liver fat trial on this page, so I cannot give you a number for that.

The real world reports are from grey market users on r/peptides, r/PeptidesUK and r/Retatrutide. They are not trial results and they are not the Chinese product.
My bottom line
This is a real, approved drug in China with real trial results behind it. Outside China it is not approved, and the vials sold online are not the Chinese product. I would not put a grey market vial of it in my body.

What is Mazdutide?
Mazdutide is a weekly shot that works on GLP-1 and glucagon. China approved it in 2025 for obesity and type 2 diabetes, the first drug of its kind approved anywhere. The FDA has not approved it.
Mazdutide is a once a week shot that works on two signals, GLP-1 and glucagon. It is based on oxyntomodulin, a natural gut hormone that works on both. It has been studied for obesity and type 2 diabetes, in large final-stage studies in China. In 610 Chinese adults with extra weight, people lost 10.1% of their weight on the lower dose and 12.6% on the higher dose over 32 weeks. People on the dummy shot gained 0.5%. In 731 Chinese adults with type 2 diabetes, both doses lowered blood sugar more than dulaglutide, an older diabetes drug, and took off 3.8 to 5.8 percentage points more weight.
Update, September 2026: China approved mazdutide in June 2025 for obesity and in September 2025 for type 2 diabetes. It was the first GLP-1 and glucagon drug approved anywhere. It is not FDA-approved.
How does Mazdutide work?
It switches on the GLP-1 and glucagon receptors. That turns down appetite, helps blood sugar and makes the body burn more energy.
What have studies in people found about Mazdutide?
Mazdutide has finished its large final-stage trials and has two approvals. Every main trial was run in China, in Chinese adults. About 2,600 people have taken it in the published trials, for obesity and type 2 diabetes, for up to 60 weeks.
- Against an older drug in type 2 diabetes: 731 adults. Both mazdutide doses lowered three-month average blood sugar more than dulaglutide 1.5 mg. People on mazdutide also lost 3.8 to 5.8 points more weight at 28 weeks.
- Obesity: 610 adults lost 10.1% of body weight on 4 mg and 12.6% on 6 mg by week 32. People on a dummy shot gained 0.5%.
- The top 9 mg dose: 461 people with obesity at 27 hospitals were treated for 60 weeks, two on the drug for every one on a dummy shot. The drug group lost 16.65% of body weight against 1.50% on the dummy. 84.3% lost at least 5%, against 33.1%. Vomiting hit 53.1% against 1.3%, nausea 46.9% against 3.2%, and diarrhoea 39.4% against 6.5%. 2.9% quit over side effects, against none on the dummy.
- Type 2 diabetes, mazdutide alone: 320 people took it or a dummy shot for 24 weeks, then 24 more weeks. Three-month average blood sugar fell 1.57 points on 4 mg and 2.15 on 6 mg, against 0.14 on the dummy. Weight fell 5.61% and 7.81%, against 1.26%.
- An earlier mid-stage diabetes trial: 250 adults over 20 weeks. Three-month average blood sugar fell 1.41 to 1.67 points on mazdutide and 1.35 on dulaglutide 1.5 mg. It rose 0.03 on a dummy shot. Weight fell up to 7.1%, more at higher doses. Diarrhoea hit 36%, low appetite 29%, nausea 23% and vomiting 14%. Low blood sugar hit 10%, against 8% on the dummy.
What this does not tell you: Every person in every main trial was recruited in China. In the 9 mg trial, people averaged 94.0 kg and 33.9 years old at the start. That is lighter and younger than a typical Western obesity trial, so the percentages may not carry over. There is no heart or kidney outcome trial and no data on deaths. The longest anyone took it was 60 weeks. Stomach side effects at 9 mg are heavy: more than half the people vomited. And mazdutide is not approved by the drug regulators of the US, Europe, the UK or Australia. So nobody outside China is taking a product made and supervised the way the trial drug was.
The state of the research
Mazdutide corrects a common mistake: thinking that little Western evidence means little evidence. The drug is licensed to Innovent Biologics for Greater China, and the trial money follows the licence. That is why a drug with two national approvals, finished large trials and more than 2,600 people in controlled trials has almost no data on anyone outside China. It is also why its trials carry names like GLORY and DREAMS, not the Western trial names people know.
This matters for you in a specific way. Percentage weight loss does not travel well. It depends on starting weight, body make-up, age and usual diet. The 9 mg trial enrolled people who averaged 94.0 kg and 33.9 years old. Reading its 16.65% loss at 60 weeks as if it came from the same kind of people as the big Western tirzepatide trial is a mistake. The trials do not support that in either direction, and no study bridging the two groups has been published.
There is no real science argument about whether the drug lowers weight and blood sugar. The trials are large and agree with each other. The open questions are the ones a regional drug programme does not pay for. What does it do to heart attacks, strokes and kidney failure? What happens after 60 weeks? How does a drug that also works on the glucagon receptor behave in people with different starting health risks? Meanwhile mazdutide is sold in the United States as a 'research chemical'. The people buying it would not have qualified for any trial that produced these numbers. They are relying on an approval from a regulator whose review papers most of them cannot read.

Mazdutide caused substantial weight loss in a large late stage trial. Six hundred and ten Chinese adults carrying extra weight were given one of two doses or a dummy injection once a week for about eight months. The lower dose group lost 10.1% of body weight and the higher dose group 12.6%. The dummy group gained 0.5%. For someone of 90 kg the higher dose is about 11 kg lost. Everyone here was Chinese, so other populations are a separate question.
Phase 3 in 610 adults: mean weight change of minus 10.1% (4 mg) and minus 12.6% (6 mg) versus plus 0.5% on placebo at week 32.

Head to head against an established diabetes drug, mazdutide won on both blood sugar and weight. Seven hundred and thirty-one Chinese adults with type 2 diabetes were given mazdutide at one of two doses or dulaglutide for about seven months. Both mazdutide doses lowered average blood sugar more than dulaglutide did. They also took off 3.8 to 5.8 percentage points more body weight. The dulaglutide dose used was 1.5 mg, which is not the highest dose that drug is licensed at.
Phase 3 in 731 adults: both mazdutide doses were superior to dulaglutide 1.5 mg for HbA1c and produced 3.8 to 5.8 percentage points more weight loss at 28 weeks.

Mazdutide produced large weight loss at its highest dose, and made a lot of people vomit. 461 adults at 27 Chinese hospitals got the weekly shot or a dummy one for 60 weeks, two to one. Nobody involved knew which. The drug group lost 16.65% of their body weight against 1.50% on the dummy, and 84.3% lost at least a twentieth of their weight against 33.1%. Vomiting hit 53.1% against 1.3% and nausea 46.9% against 3.2%.

Mazdutide beat a dummy shot on blood sugar and edged an older drug, in a short mid-stage trial. 250 Chinese adults with type 2 diabetes were treated for 20 weeks. Average blood sugar over three months fell between 1.41 and 1.67 points across mazdutide doses, against 1.35 on dulaglutide and no change at all on the dummy. Weight fell up to 7.1%. Low blood sugar was reported by 10% against 8% on the dummy. Twenty weeks is a short look.
250 adults over 20 weeks: HbA1c -1.41% to -1.67% across mazdutide doses versus -1.35% on dulaglutide and +0.03% on placebo, with weight change up to -7.1%. Hypoglycaemia 10% versus 8% on placebo.
