Orforglipron (oral non-peptide GLP-1 agonist)

Overview

Orforglipron is an oral GLP-1 receptor agonist (FDA-approved April 2026 as Foundayo) notable for being a small-molecule pill rather than an injected peptide, which could make GLP-1 therapy easier to scale. Phase 2 and Phase 3 trials have reported meaningful weight loss and glucose lowering. It is included here because it competes directly with the peptide GLP-1 drugs. Update, September 2026: the FDA approved orforglipron on April 1, 2026 under the brand name Foundayo, about 10 weeks after the NDA was received on January 20, 2026, under the National Priority Voucher program, making it the first oral small-molecule GLP-1 agonist. ATTAIN-1 reported 12.4% weight loss at 72 weeks on the 36 mg dose by the efficacy estimand (11.2% versus 2.1% for placebo in the NEJM treatment-regimen analysis).

Mechanism of action

Activates the GLP-1 receptor as an orally available small molecule, reducing appetite and improving glucose-dependent insulin secretion.
