This is how I would explain it to a friend. The evidence, with the numbers, is further down the page.
VIP is a natural signalling peptide. It calms swelling, tunes the immune system and opens up blood vessels. The natural form lasts only a minute or two in the body. Some clinics use it as a nose spray, made by compounding pharmacies. It is not broadly FDA-approved.
What people take it for
- A long-running swelling illness called chronic inflammatory response syndrome.
- High blood pressure in the lungs.
- Calming an overactive immune system.
What studies in people showed
The biggest study had 196 people with critical COVID lung failure. They got VIP into a vein or a dummy. By day 60, the share alive and off breathing support was no better on the drug. A second look at survival alone leaned its way, but that is a lead, not a result. In a small early study, people with high blood pressure in the lungs breathed it in. Their lung pressures and walking improved. Everyone knew what they got, and there was no dummy group. There is no good study of the nose spray on this page.

Vasoactive Intestinal Peptide (VIP) is a signalling peptide 28 building blocks long. It affects the immune system, swelling, how wide blood vessels open, and the body clock. The biggest human study had 196 people with COVID lung failure. They got VIP (as aviptadil) into a vein or a dummy. By day 60, the share alive and off breathing support was no better on the drug. A second look at survival alone leaned its way, but that is a lead, not a result. In a small early study, people with high blood pressure in the lungs breathed it in. Their lung pressures and walking improved, but everyone knew what they got. It has also been used as a nose spray for a long-running swelling illness called chronic inflammatory response syndrome, with little good evidence. It is used off label and made by compounding pharmacies in some places. It is not broadly FDA-approved.
How does VIP work?
It switches on VPAC receptors. That calms immune and swelling signals, relaxes smooth muscle and opens up blood vessels.

A small, early study in a serious lung condition, with everyone knowing what they were getting. People with high blood pressure in the lungs breathed in VIP. Pressures in the lung circuit improved and they could walk further. All of them were short of VIP to begin with, which is the best case there is for giving it back. There was no dummy group and nobody was kept in the dark, so how much of this was the drug is unknown.
Small open study: inhaled VIP improved hemodynamics and exercise capacity in primary pulmonary hypertension patients deficient in VIP.

The trial missed the thing it was built to answer. A hundred and ninety six people with critical COVID lung failure were given aviptadil into a vein or a dummy. By day 60, the share who were alive and off breathing support was no better on the drug. A separate look at survival alone leaned the drug's way. That was not the question the trial set out to settle, and a result found by looking again is a lead, not a finding.
196 patients: primary endpoint (alive and free of respiratory failure at day 60) not met; survival odds favored aviptadil in secondary analysis.
