What is BPC-157?

How does BPC-157 work?

In animals, BPC-157 helps grow new blood vessels. It works with nitric oxide, a gas the body uses to widen blood vessels. In tendon cells from animals, it raises growth hormone receptors and growth signals. Nobody has found the exact switch it acts on. The FDA's 2026 review gave that as one reason it could not judge whether it should work. In people, very little is known. After an enema, it could not be found in the blood. A two-person test suggests it leaves the blood in under 30 minutes. Animal studies describe effects that last hours to days.

What have studies in people found about BPC-157?

Five published studies have given BPC-157 to people, with fewer than 110 people in all. The FDA's July 2026 review lists all of them. Only one compared it with a dummy treatment. That was a two-week enema study in ulcerative colitis. The BPC-157 group did a bit better, but the gap was small enough to be chance. The rest are one early safety test and three clinic reports with nobody to compare against.

• Ulcerative colitis, a bowel disease, run at several hospitals (Ruenzi and others, 2005, a meeting talk described in the FDA's 2026 review): 53 adults with mild to moderate disease got an 80 mg BPC-157 enema (code PL 14736) or a dummy enema once a day for two weeks. Nobody knew who got which. 46 finished. On a disease score, the BPC-157 group dropped 3.2 points on average and the dummy group dropped 1.6. The 1.6 point gap between them was small enough to be chance. Three people on BPC-157 and two on the dummy quit because of problems, mostly their colitis getting worse.
• Early safety test in healthy people, Croatia (Veljaca and others, 2002 and 2003, meeting talks described in the FDA's 2026 review): 32 people took part, and 24 got BPC-157 as an enema at 0.25, 0.5, 1 or 2 mg/kg. They had one dose, then a week off, then seven daily doses. The authors saw no serious side effects from the peptide. Headache and gas happened about as often on the dummy. BPC-157 could not be found in the blood.
• Knee pain, one Florida clinic, 2019 to 2020 (Lee and Padgett 2021): doctors looked back through old records. 17 patients had 2 to 4 mg injected into the knee, in 1 or 2 doses. 16 were reached by phone 6 to 12 months later. Of the 12 who got BPC-157 alone, 11 said their knee pain was much better. Of the 4 who also got thymosin beta-4, 3 said so. People rated their pain from memory. There was no comparison group, no standard pain score and no scans.
• Interstitial cystitis, a long-lasting bladder pain problem, one clinic (Lee and others, 2024): 12 women aged 39 to 76 had not been helped by the usual drug, pentosan polysulfate. Each got 1 mg injected in 10 spots in the bladder wall, 10 mg in all, during one scope procedure. On a symptom rating, 10 of 12 said their symptoms were fully gone and 2 of 12 said about 80% gone. Nobody dropped out and no side effects were reported. There was no dummy group, and everyone knew what they got.
• Drip into a vein, safety test (Lee and Burgess 2025): 2 healthy adults got 10 mg into a vein over one hour on day 1 and 20 mg on day 2. They reported no side effects. Heart rate, blood pressure and blood tests did not change in any way that mattered. The blood tests covered blood count, body chemistry, a muscle marker, a heart marker, thyroid, magnesium and a swelling marker.
• Signed up but never reported: NCT02637284 planned to test BPC-157 tablets in 42 healthy people in Mexico. It has posted no results and has not been updated since December 2015. NCT07752381 gave a 500 mcg BPC-157 gummy to 40 people for 8 weeks, with everyone getting the gummy. It measured two blood markers of swelling, hs-CRP and IL-6. It finished in November 2025 and has posted no results.
• Signing people up now: NCT07437547 is testing 120 people with a fresh grade II hamstring tear. Each gets a daily shot under the skin of BPC-157 or a dummy for 14 days, and nobody knows which. It measures the time until they are back to full sport, and the size of the tear on an MRI scan at day 14. It is expected to finish in February 2027. NCT07803250 plans to test 30 patients after rotator cuff surgery at the University of Arkansas. It is not signing people up yet and is set to start in 2027.

What this does not tell you: Nothing here shows BPC-157 fixes a tendon, ligament or joint in a person. The one study with a dummy group tested an enema in ulcerative colitis, and its result fits with no effect at all. The knee and bladder reports had nobody to compare against, and everyone knew what they got. The knee report looked back at old records and asked people by phone. All three clinic reports come from the same group of doctors. No published study has used a shot under the skin or a pill, which is how buyers take it. Nobody has worked out a dose. Nobody has measured how it moves through the body, except in a two-person test. Nobody in a study has taken it for more than two weeks, so the record says nothing about months of use.

What do people who took BPC-157 report?

This is what real people say happened to them, good and bad. Nobody measured it. A report cannot tell a real effect from the lift people get just from expecting it to work, from a diet or training change made at the same time, or from what was really in the vial. We still put it near the top. It is the next best thing to a study in people: what happens when real people use it, including what goes wrong.

• Doses people describe online are mostly 250 to 500 mcg as a shot under the skin, once or twice a day, for four to eight weeks, often near the injury. The strengths pharmacies proposed to the FDA match that: 250 mcg, 500 mcg and 1 mg capsules, and a 2,000 mcg/mL shot. Clinic use in published reports looks different again: one dose of 2 to 4 mg into a joint, or 10 mg into the bladder wall.
• People say they use it for tendon pain, mostly elbow, shoulder and Achilles, for partly torn muscles, for knee pain and after surgery. Some use it for gut trouble such as reflux and bowel swelling.
• The usual story is less pain in the first one to two weeks, then the joint or tendon handling more load by weeks three to six. People who stop before about four weeks often say nothing lasted.
• Taking it with TB-500 is the most common plan. It is sold and talked about as a healing stack. That means nobody can tell from a user report which peptide did what. It is also what was going on in the one side effect report the FDA judged likely caused by the drug.
• No effect is a common report. Forum logs include people who took a milligram or more of each peptide daily for four weeks and saw no change in pain or swelling. Cost, often a few hundred dollars per round, is a reason people stop.
• The problems people name most are redness, swelling and lumps where the needle went in, and not knowing what is really in the vial. The FDA's side effect database has three reports through December 2025 for injected BPC-157. A 55-year-old woman, who was also injecting compounded thymosin, had nine days of redness and swelling at the shot site. A 28-year-old man went to the emergency room short of breath. A 40-year-old woman using a research-labelled BPC-157 plus TB-500 product had her skin and gums darken. It went away when she stopped and came back the same way when she started again. The FDA notes that with two peptides in one product, that case cannot be pinned on either one.
• Where to buy it and whether it is pure take up most of the practical talk. People discuss research-use labels, lab reports of unknown origin and paid purity tests. They argue about whether a failed round means the peptide does not work or the vial did not hold what the label said. Reviews by bone and joint doctors make the same point from the other side. They warn that unregulated making and contamination are the main safety worry.
• Some people report new or worse tendon or joint pain while taking it. These reports are much rarer than reports of relief. No published study has looked for this on purpose.

  Intra-Articular Injection of BPC 157 for Multiple Types of Knee Pain

  This is not a trial, and it cannot show the peptide helped anyone. Doctors at one Florida clinic looked back through the records of 17 patients who had been injected in the knee. They phoned 16 of them six to twelve months later and asked how the knee felt. Of the 12 given BPC-157 on its own, 11 said the pain was meaningfully better. Nobody got a dummy injection, nobody was scored on a proper pain scale, and no scans were taken.

  Retrospective chart review of 17 patients given 2 to 4 mg intra-articularly at one Florida clinic; of the 16 reached by phone months later, 11 of 12 who received BPC-157 alone reported meaningful pain relief. No control group, no validated outcome measure, no imaging.

  Effect of BPC-157 on Symptoms in Patients with Interstitial Cystitis: A Pilot Study

  Almost every woman in this small study reported her symptoms gone, and there was nothing to compare them against. All of them had already failed the standard drug for interstitial cystitis. Each had 10 mg of BPC-157 injected into the bladder wall during one procedure. Ten of the twelve later reported the symptoms had cleared completely, and the other two reported about four fifths gone. Everybody got the peptide. Nobody was kept in the dark, and nobody went without.

  Single-arm study in 12 women who had not responded to pentosan polysulfate: 10 mg injected into the bladder wall in one procedure, 10 of 12 reported complete symptom resolution and 2 of 12 about 80% resolution. No placebo group and no blinding.

  Safety of Intravenous Infusion of BPC157 in Humans: A Pilot Study

  A study this small cannot show that anything is safe. Two healthy adults were given BPC-157 into a vein, 10 mg on the first day and 20 mg on the second. Neither reported side effects, and blood tests covering the heart, liver, kidneys, thyroid and blood sugar did not shift. That is worth knowing, and it is all it is. Two people cannot reveal a side effect that strikes one person in fifty, let alone one in a thousand.

  Two adults received 10 mg intravenously on day 1 and 20 mg on day 2. No adverse effects were reported and cardiac, liver, kidney, thyroid and glucose laboratory values did not change meaningfully. Two subjects cannot characterise safety.

  Nobody has ever reported what happened in this study. A company registered a plan in 2015 to give healthy volunteers BPC-157 tablets, first as single doses and then three times a day for two weeks, with 42 people intended. No results were ever posted. The record has not been touched since December 2015 and its status is listed as unknown. A registration is a statement of intent, not evidence, and this one may never have run at all.

  Randomized, Double-Blind, Placebo-Controlled Phase 2 Trial of BPC 157 for Acute Grade II Hamstring Strain (NCT07437547)

  This is the first proper test of BPC-157 on an injury, and it is still running. 120 people with a torn hamstring get a daily injection under the skin of either BPC-157 or a dummy version for two weeks, on top of normal rehab. Nobody involved knows which. The trial times how long each person takes to get back to full sport, and scans the muscle after two weeks. Recruiting began in February 2026. First results are expected in 2027.

  The first controlled musculoskeletal trial: 120 participants, daily subcutaneous injection or placebo for 14 days on top of standard rehabilitation, co-primary endpoints time to unrestricted sport and MRI injury volume at day 14.   A Clinical Trial to Evaluate the Effects of Peptide Gummies on Markers of Inflammation, Physical Performance, and Recovery (NCT07752381)

  A company registered a study of a BPC-157 gummy, marked it completed, and has posted no results. 40 physically active adults were to take two gummies a day for eight weeks, 500 mcg of BPC-157 per serving. The main measurements were two blood markers of inflammation, hs-CRP and IL-6. Everybody was to get the gummy. There was no dummy gummy to compare against.

  Impact of BPC-157 on Recovery Following Rotator Cuff Repair Surgery (NCT07803250)

  This trial has not started. The University of Arkansas has registered a plan to give people BPC-157 or saline after rotator cuff repair, as a daily shot under the skin for 90 days. 30 people are planned. First dosing is listed for 2027. A registration is a statement of intent, not a result.

  A review of everything published on BPC-157. It found many effects in animal studies of injuries, bowel disease and the brain. It notes that no regulator has approved it, because there are not enough studies in people.

  Gastric pentadecapeptide BPC 157 accelerates healing of transected rat Achilles tendon and in vitro stimulates tendocytes growth

  In rats with a cut Achilles tendon, a daily dose of BPC-157 made the healing tendon stronger than salt water did. The rats also walked better, and more collagen, the tendon's building protein, formed.
