This is how I would explain it to a friend. The evidence, with the numbers, is further down the page.
Cagrilintide is a once a week shot that makes you feel full sooner. It copies a fullness hormone called amylin that your body makes along with insulin. Ozempic works on a different hunger signal, so the two are being tested together as CagriSema. On its own it is not approved anywhere. Anything sold online as cagrilintide is not the drug from the trials.
What people take it for
- Weight loss with less nausea than the Ozempic type drugs.
- Stacking it with Ozempic or Mounjaro at home to copy CagriSema.
- Getting past a weight loss stall on a GLP-1 drug.
- Feeling full on smaller meals.
What studies in people showed
On its own, 706 people took it for 26 weeks in a dose finding trial. They lost 6.0 to 10.8 percent of their body weight depending on the dose. The dummy shot group lost 3.0 percent. The top dose beat daily liraglutide, 10.8 percent against 9.0 percent. With Ozempic added, in 92 people with diabetes over 32 weeks, the pair lost 15.6 percent. Ozempic alone lost 5.1 percent and this drug alone lost 8.1 percent in that trial. In the large final stage trial of the pair, 1,206 people with diabetes lost 13.7 percent in 68 weeks against 3.4 percent on the dummy shot. That is a strong result. But it is a result for the pair. The only trial of this drug by itself on this page is the early dose finding one. And nobody has tested mixing two grey market vials at home.

The bad
- Nausea, mainly at the start and at dose jumps.
- Red, itchy or sore spots where the needle went in. This comes up a lot in the reports.
- Grey market vials with no way to know what is in them or how strong they are.
- Tiredness, and an appetite that drops too far for some people.

My bottom line
The pair with Ozempic is the real story, and that has its own page. On its own this drug has only had early trials. It is not approved, and what is sold online is not the trial drug. I would not put a grey market vial of it in my body, and I would not stack two of them.

What is Cagrilintide?
Cagrilintide is a weekly shot being tested with semaglutide as a pair called CagriSema. Together they took off more weight than either one alone. It is not approved.
Cagrilintide is a weekly shot that copies amylin, a hormone that helps you feel full. It is being tested with semaglutide as one combined shot called CagriSema. The biggest study had 1,206 adults with extra weight and type 2 diabetes. After 68 weeks, they lost 13.7% of their body weight on CagriSema and 3.4% on a dummy shot. In 706 adults with extra weight, cagrilintide on its own took off 6.0% to 10.8% in six months, depending on the dose. The dummy shot took off 3.0%. A smaller study in 92 people with type 2 diabetes tested each drug alone and the pair together for 32 weeks. The pair took off 15.6% of body weight. Cagrilintide alone took off 8.1%. Semaglutide alone took off 5.1%. It is still being tested and is not approved.
How does Cagrilintide work?
It works on amylin receptors in the parts of the brain that control hunger. That helps you feel full and slows how fast food leaves your stomach. This adds to what GLP-1 drugs like semaglutide do.
What have studies in people found about Cagrilintide?
Nearly everyone who has taken cagrilintide took it as half of a combination with semaglutide. Only one trial tested it on its own: a 26-week dose-finding study in 706 people. The large final-stage trials that followed tested the combination. Cagrilintide alone was only a small comparison group in them.
- The biggest trial: 3,417 adults, mostly on the combination. Only 302 got cagrilintide 2.4 mg alone and 302 got semaglutide 2.4 mg alone. The combination group lost 20.4% of body weight at 68 weeks, against 3.0% on a dummy shot. Stomach and gut side effects hit 79.6% against 39.9%.
- Type 2 diabetes: 1,206 adults on the combination lost 13.7% at 68 weeks, against 3.4% on a dummy shot. 73.5% got their three-month average blood sugar to 6.5% or lower.
- The only trial of cagrilintide on its own: 706 people over 26 weeks on doses from 0.3 to 4.5 mg. They lost 6.0% to 10.8% of body weight, against 3.0% on a dummy shot. The top dose did a little better than liraglutide 3.0 mg, an older weight drug: 10.8% against 9.0%. Stomach and gut side effects hit 41% to 63%, against 32% on the dummy.
- Against semaglutide alone: 331 people in Japan and Taiwan got the combination or semaglutide 2.4 mg by itself, not a dummy. They lost 18.4% against 11.9% at 68 weeks. That 6.5-point gap is the cleanest measure of what cagrilintide adds on top of semaglutide.
- A small diabetes trial: 92 adults with type 2 diabetes over 32 weeks lost 15.6% on the combination, 8.1% on cagrilintide alone and 5.1% on semaglutide alone.
What this does not tell you: Only one trial has asked what cagrilintide does on its own. It ran 26 weeks and was built to pick a dose, not to test real health results. Every trial since asks a different question: what does it add to semaglutide? No trial of cagrilintide alone has tracked heart attacks, kidney problems or deaths, and none is planned. Nothing is known about taking it past 68 weeks, about weight coming back after stopping this drug, or about its effect on muscle and fat. It is not approved in any country. Its drug family, which copies the hormone amylin, also has a history. Pramlintide, the first approved drug of this type, must be injected several times a day and has never caused weight loss this large. Cagrilintide's long-lasting design is the new part being tested, and it has been tested for barely a year.
What do people who took Cagrilintide report?
This is what real people say happened to them, good and bad. Nobody measured it. A report cannot tell a real effect from the lift people get just from expecting it to work, from a diet or training change made at the same time, or from what was really in the vial. We still put it near the top. It is the next best thing to a study in people: what happens when real people use it, including what goes wrong.
- It is discussed almost entirely as something added on top of a GLP-1 rather than as something used alone, and the question people actually ask each other is when to add it, not whether it does anything.
- The stated reason for adding it is that the GLP-1 drug alone did not cut appetite enough. Posters describe it as working a different way, not as simply more of the same drug.
- The striking thing about these threads is how little experience is in them. Questions outnumber answers, no doses are reported by anyone describing their own use, no side effect accounts appear, and the more experienced posters describe themselves as uncertain about how to combine it. That absence is the finding, and it means nothing here should be read as evidence about what the compound does.

The state of the research
Cagrilintide was never meant to be sold alone. Novo Nordisk built it as one half of CagriSema. That business plan explains the shape of the evidence. A company planning to sell a fixed combination has no reason to pay for large trials of what the smaller part does alone. So the solo data stop at 706 people and 26 weeks. The 3,417-person trial gave cagrilintide alone to 302 people only as a yardstick, not as a real test of it as a treatment.
That matters if you read the solo numbers as if they describe a drug you might take by itself. The 6.0% to 10.8% range at 26 weeks is real, and it came from a well-run trial. But it is a dose-finding result. Those trials exist to pick a dose for the next study, not to describe a treatment. The number that really describes cagrilintide's part comes from the trial in Japan and Taiwan. Adding it to semaglutide 2.4 mg moved 68-week weight loss from 11.9% to 18.4%, an extra 6.5 points.
Nobody seriously doubts that drugs copying amylin make people eat less. The live contest is a business one: several companies are building their own amylin drugs, and each wants to win. That contest is the reason cagrilintide has large trial data at all. Read the evidence here as evidence about a combination, because that is what was tested.

Adding cagrilintide to semaglutide roughly doubled weight loss in a small early trial. 92 adults with type 2 diabetes took the pair, or semaglutide alone, or cagrilintide alone, for eight months. The pair took off 15.6% of body weight. Semaglutide alone took off 5.1% and cagrilintide alone 8.1%. This was an early study in fewer than a hundred people, all of them diabetic. It is far too small to tell you how often the side effects happen or who they happen to.

Cagrilintide on its own took off weight, and its top dose edged out an approved drug. 706 adults carrying extra weight took cagrilintide at one of several doses, a dummy injection, or liraglutide, for six months. Weight fell between 6.0% and 10.8% depending on dose, against 3.0% on the dummy. At 4.5 mg it beat liraglutide, 10.8% against 9.0%. This was a study to find the right dose, not to settle the question. Six months tells you nothing about keeping weight off.
706 participants over 26 weeks: cagrilintide monotherapy produced 6.0% to 10.8% weight loss across doses vs 3.0% with placebo; 4.5 mg beat liraglutide 3.0 mg (10.8% vs 9.0%).

In a large late-stage trial the pair took off substantial weight and pulled most people's blood sugar into range. 1,206 adults with extra weight and type 2 diabetes took CagriSema or a dummy injection for about sixteen months. Body weight fell 13.7% on the drug and 3.4% on the dummy. Nearly three quarters got their average blood sugar down to 6.5% or lower. Everyone in this trial had diabetes. Sixteen months is a long trial and still says nothing about a decade.
Phase 3a, 1,206 patients: -13.7% body weight at 68 weeks with CagriSema vs -3.4% with placebo; 73.5% reached HbA1c of 6.5% or lower.

The combination of cagrilintide and semaglutide produced some of the largest weight loss yet published. 3,417 adults were split by chance between the combination, either drug alone and a dummy shot, with 302 given cagrilintide by itself. Nobody involved knew which. After 68 weeks the combination group had lost 20.4% of their body weight against 3.0% on the dummy. Stomach and bowel side effects hit 79.6% on the combination against 39.9%. Almost nobody in this trial took cagrilintide alone.
REDEFINE 1, 3,417 participants randomized 21:3:3:7, including 302 to cagrilintide 2.4 mg alone: the combination produced -20.4% at 68 weeks versus -3.0% on placebo, with gastrointestinal adverse events in 79.6% versus 39.9%.

This is the cleanest measure of what cagrilintide actually adds. 331 people in Japan and Taiwan got either the combination or semaglutide by itself, so the comparison was against a working drug rather than a dummy. After 68 weeks the combination group had lost 18.4% of their body weight and the semaglutide group 11.9%. Cagrilintide is worth about 6.5 percentage points on top. It was measured in an east Asian population, which may not transfer.
331 participants: -18.4% on the combination versus -11.9% on semaglutide 2.4 mg alone at 68 weeks, an estimated treatment difference of 6.5 percentage points. The clearest published measure of cagrilintide's incremental contribution.
