What is the difference between time on time off and continuous approved drugs?

Time on, time off is a user-chosen calendar. Continuous approved drugs are taken for as long as the labeled condition is being treated. Semaglutide, tirzepatide, and tesamorelin are approved as continuous regimens with no off period on the label. That is a regulatory fact about those products, not a finding that every unapproved peptide should be run the same way, and not a finding that a cycle is required for the ones that were never labeled.
