This is how I would explain it to a friend. The evidence, with the numbers, is further down the page.
Sermorelin is a shot that nudges the gland in your brain to release its own growth hormone. It is a copy of the first 29 pieces of the release hormone your brain already makes. It used to be an approved drug, Geref, for testing and treating kids who could not make growth hormone. That product was pulled from the market. Now it is made by compounding pharmacies and sold by anti-ageing clinics.
What people take it for
- Bringing growth hormone back up as they age.
- Better sleep and more energy.
- A leaner body and better skin.
- A gentler option than growth hormone shots, because the body still controls the pulses.
What studies in people showed
The main adult study had 10 healthy older men. They took 1 mg twice a day for 14 days. Their growth hormone and IGF-1 over 24 hours came back up to about the level of young men. That is a blood test result, not a body result. It did not measure muscle, fat, sleep or energy. Ten men for two weeks is the adult evidence. In kids who could not make growth hormone, a daily dose of 30 micrograms per kilo helped some of them. A single test dose was used to diagnose the problem. Nobody has tested whether it makes healthy adults look or feel younger.

Where these come from: There are no trial exit interviews for adults. The reports above are from user forums, mostly r/Peptides, and from clinic and telehealth write-ups that sell it. Clinic write-ups have a reason to talk it up, so read them that way.
My bottom line
It does raise growth hormone, and a pharmacy version on a prescription beats a grey market vial. But ten men for two weeks is the adult evidence. I would not spend money on it before my bloodwork and my training were sorted, and I would not expect it to do more than the trial did.

What is Sermorelin?
Sermorelin is a piece of the hormone that tells your body to make growth hormone. It was once approved for children short on growth hormone. Now it is made by compounding pharmacies and used for anti-aging.
Sermorelin is a lab-made copy of the first 29 building blocks of GHRH. GHRH is the hormone that tells your pituitary gland to release growth hormone. This short piece is the smallest part that still fully works. It was once FDA-approved as Geref. Doctors used it to test for growth hormone shortage and to treat children who did not make enough. The test dose was 1 microgram per kilogram into a vein. The treatment dose for children who had not reached puberty was 30 micrograms per kilogram a day. That product was stopped, and now it is only made by compounding pharmacies. It does not replace growth hormone. It makes your body release its own in natural bursts. In ten healthy older men, 1 mg twice a day for 14 days brought growth hormone and IGF-1 back to the levels of young men.
How does Sermorelin work?
It switches on the GHRH receptor in the pituitary gland. That makes the gland release growth hormone the normal way.
What have studies in people found about Sermorelin?
Sermorelin was an approved drug and has real trial data. Almost all of it was collected before 2000, and almost all in children with growth hormone deficiency. The largest study treated 110 children for up to a year with no comparison group. Nothing published tests it for the adult use it is now compounded and sold for.
- Children: 110 young children with growth hormone deficiency who had never been treated injected 30 micrograms/kg under the skin at bedtime for up to a year. There was no comparison group, and 86 of the 110 could be counted for results. Their growth rate rose from 4.1 cm a year to 8.0 at 6 months and 7.2 at 12 months. 74% responded well at 6 months. Bone age moved in step with height (a ratio of 1.04). IGF-1 did not overshoot and fasting blood sugar did not change.
- Adults aged 55 to 71, on a close cousin of sermorelin rather than sermorelin itself: 10 women and 9 men injected salt water nightly for 4 weeks, then the peptide at 10 micrograms/kg for 16 weeks. Night-time growth hormone rose in both sexes. IGF-1 and a related protein rose within 2 weeks, stayed up for 12 weeks, then drifted back toward the start by week 16 even though dosing went on. Lean body mass rose in men only. Skin got thicker in both. Bone density did not change in either. Insulin response, well-being and sex drive improved in men only. Sleep did not change in anyone. A short-lived rise in blood fats was the only side effect reported.
- Healthy older men (already cited on this page): 10 men took sermorelin, 1 mg twice a day, for 14 days. Afterwards their 24-hour growth hormone and IGF-1 were no longer clearly different from young men's.
- Test and child treatment review (already cited): 1 microgram/kg into a vein is a quick, fairly specific test of the pituitary's growth hormone cells. 30 micrograms/kg a day under the skin treated some young children with growth hormone deficiency.
- The one adult finding nobody has followed up: the hormone response faded by week 16 while the shots carried on.
What this does not tell you: The evidence of benefit is in children with a diagnosed deficiency, with no comparison group, measured by growth rate, which has no adult version. The adult data are 19 people for 16 weeks on a close cousin of the drug, and 10 people for 14 days on sermorelin itself, both in the 1990s. No trial has tested compounded sermorelin at today's doses in the people buying it. Nothing has measured body make-up, sleep, recovery or injury healing in healthy adults over a year. No study has looked at what happens after the IGF-1 response fades.
What do people who took Sermorelin report?
This is what real people say happened to them, good and bad. Nobody measured it. A report cannot tell a real effect from the lift people get just from expecting it to work, from a diet or training change made at the same time, or from what was really in the vial. We still put it near the top. It is the next best thing to a study in people: what happens when real people use it, including what goes wrong.
- The standout in this discussion is not an effect, it is a reaction. People describe itching at the injection site progressing to flushing, hives over the body, itching of palms and soles, palpitations, shallow breathing, sweating and weakness, usually within minutes of injecting. Accounts describe this appearing after months of uneventful use, worsening with each further dose, and not being solved by changing site or vial. Most stop. The body making antibodies against the peptide is the explanation people offer each other.
- Doses described run from around 200 to 500 mcg at night up to about 1 mg daily for several months before dropping to a few times a week, and the expectation set by more experienced posters is a loading period of three to six months before judging anything.
- Disappointment is common. It is repeatedly described as weak compared with the newer growth hormone release peptides, and the frequent advice is to combine it with a GHRP, which means most reports about it are reports about a combination.
- Other negatives described include carpal tunnel type symptoms, stomach complaints when it is stacked, and isolated accounts of raised liver enzymes.
- Cost is a recurring reason for stopping, with compounding pharmacy pricing of roughly 165 dollars a vial for about a month, and cheaper research chemical suppliers raising the usual questions about what is actually in the vial.
- Among those who tolerate it, the reported effects are sleep quality first, then energy, mood and gradual body composition change, and even favourable accounts question whether that is worth the money and the time.

The state of the research
Sermorelin is the one compound in this group that was approved, worked well enough to stay approved, and left the market anyway. EMD Serono told the FDA, in letters dated 11 July and 2 December 2008, that it was stopping both versions of Geref. The FDA withdrew approval of both on 18 June 2009. In 2013, answering a citizen petition, the FDA ruled that neither product had been withdrawn because it was unsafe or did not work. The stated purpose of that ruling is to let cheaper copy versions apply for approval.
That ruling is also the door the compounding market walks through, so it is worth being exact about what it says. It says the products were not pulled for being dangerous, or for failing at their approved use. That use was poor growth in children with growth hormone deficiency. It says nothing at all about whether nightly sermorelin does anything useful for a healthy adult. That was never the approved use, never the tested use, and never put before the agency.
Nobody tested the adult use for the same money reason that runs through this whole site, plus one extra fact: man-made growth hormone already existed. A drug that makes the body release its own growth hormone might keep the body's natural pulses, which is a possible advantage. But by the late 1990s, a company faced a long trial in healthy adults, a 29-amino-acid fragment it could not patent, and a rival product already on the shelf. There was no way to earn the money back. So the trials stopped, and the product was withdrawn for business reasons. The clinics selling compounded sermorelin today are working from 1990s child data and from how the hormone works, not from any trial in patients like theirs.

A short course of injections put older men's growth hormone back to young-adult levels. 10 healthy older men injected this fragment of growth hormone releasing hormone twice a day for 14 days. Their growth hormone across a full day, and their IGF-1, rose to levels that could not be told apart from young men's. Ten men and two weeks is a very small study. It measured hormones in the blood and nothing else. Nobody checked whether the men got stronger, leaner or felt better.
In 10 healthy old men, 14 days of GHRH(1-29) 1 mg twice daily restored 24-hour GH and IGF-1 to levels not significantly different from young men.

Children with a growth hormone deficiency grew faster on sermorelin, in a study with nobody to compare them against. 110 children injected it nightly for up to a year. Their growth roughly doubled, from about four centimetres a year to eight at six months, settling near seven by a year. About three quarters responded well. Everybody got the drug and everybody knew it. Bone age advanced in step with height, and blood sugar did not change.
110 prepubertal children treated with 30 micrograms/kg per day for up to a year in an open-label multicentre study with no control arm: mean height velocity rose from 4.1 to 8.0 cm/year at 6 months and 7.2 cm/year at 12 months, with 74% classed as good responders and no excessive IGF-1 generation.

The hormone response faded while the injections carried on. Nineteen adults aged 55 to 71 injected salt water nightly for a month, then a sermorelin-like peptide for four more months. Growth hormone and its main blood marker rose within two weeks, stayed up for three months, then drifted back to where they started even though dosing never stopped. Muscle, insulin response, mood and libido improved in the men and not the women. Bone density and sleep did not change in anyone.
19 adults aged 55 to 71 on 4 weeks of placebo then 16 weeks of the Nle27 analogue at 10 micrograms/kg nightly: IGF-1 and IGFBP-3 rose within 2 weeks and returned toward baseline by week 16 while dosing continued. Lean mass, insulin sensitivity, well-being and libido improved in men only; bone mineral density and sleep quality did not change.

Sermorelin (GHRH 1-29) is the shortest fully active GHRH fragment; IV 1 mcg/kg is a rapid, relatively specific diagnostic test and daily 30 mcg/kg subcutaneous dosing treated some prepubertal children with GH deficiency.

This is the paperwork the compounded sermorelin market stands on, and it says less than it is used to say. The maker told the regulator in 2008 that it was discontinuing both versions of Geref, and approval was withdrawn in June 2009. In 2013 the agency put on record that neither product had been pulled for being unsafe or ineffective. That record is about growth failure in children, the use it was approved for. It says nothing about adults.
The regulatory record behind the compounded market: EMD Serono notified FDA in July and December 2008 that both Geref presentations were being discontinued, approval of both applications was withdrawn effective 18 June 2009, and FDA determined in 2013 that neither was withdrawn for reasons of safety or effectiveness.
