How does gonadorelin work?

Gonadorelin is synthetic gonadotropin-releasing hormone, the hypothalamic signal that drives pituitary LH and FSH. Half-life on this site's profile is about 2 to 4 minutes. Molecular weight is 1,182.3 daltons. It must be given in pulses: continuous exposure desensitizes the pituitary and shuts hormone release down, which is how drugs like leuprolide suppress the axis. In six men with hypogonadotropic hypogonadism, pulsatile subcutaneous GnRH by pump normalized gonadotropins within a week, raised testosterone from 77 to 520 ng/dL in one month, and produced spermatogenesis in three of the six. Prior FDA approvals for diagnostic and fertility uses have been largely discontinued; compounding and off-label TRT support are a different pathway from those discontinued products.
