How does rapamycin work?
Rapamycin, also called sirolimus, inhibits the mTOR pathway. This site's profile says that mimics aspects of caloric restriction and enhances autophagy and cellular maintenance. Half-life is about 60 hours. Molecular weight is 914.2 daltons. It is FDA approved as Rapamune to prevent transplant rejection; longevity use is off-label and investigational. PEARL found 48 weeks of low-dose intermittent rapamycin relatively safe in normative-aging adults, with lean mass and pain improved in women on 10 mg, one finding among many measures. Kraig 2018 randomized 25 generally healthy adults aged 70 to 95 to 1 mg daily or placebo; 11 on rapamycin and 14 controls completed at least 8 weeks. That was a feasibility and safety pilot, not a lifespan trial. No randomised trial has tested whether rapamycin extends human life. Rapamycin fed late in life extended lifespan in genetically heterogeneous mice: 14% in females and 9% in males at the 90th percentile. That is a mouse finding and it stays one.
