How does larazotide work?

Larazotide acetate, also listed as AT-1001, is an orally active peptide that this site's profile describes as a zonulin antagonist: it is meant to tighten intestinal tight junctions and limit passage of gluten fragments. Half-life is listed as short; it acts locally in the gut. Molecular weight is about 732 daltons. In 184 people with coeliac disease on a gluten challenge, 1 mg limited gluten-induced symptoms and blunted the rise in anti-tTG antibodies, but intestinal permeability (LAMA ratio) did not differ from placebo, so the barrier mechanism it was built on was not demonstrated there. Phase 2b in 342 adults found only the 0.5 mg dose met the primary symptom endpoint; higher doses did not.
